Specifications include, but are not limited to: The Contractor shall provide the following: 1.3.1 Deliverables, Performance Measures, and Monitoring Activities. 1.3.1.1 Inpatient: ICF/ID The Contractor shall provide consultation services. Services shall include: Throughout the provision of these services, Contractor shall meet the laws, rules and requirements of national and state regulators that GRC is subject to, including but not limited to: Centers for Medicare and Medicaid Services, Iowa Board of Pharmacy, and other federal and state regulatory agencies as applicable. The Contractor shall maintain and provide copies of a current, valid Iowa pharmacy license and appropriate current, valid licensure of all employees. The Contractor shall ensure that either a pharmacist or a service is available 24/7 for consultation with nurses and doctors during and after hours, weekends, and holidays. 1.3.1.1.1 Inpatient Deliverables Contractor shall provide the following: A. Staffing and Availability: 1. Provide pharmacist(s) as needed/required. Contractor’s pharmacist or a comparable service shall be available: B. Documentation and Reports: 1. Maintain necessary documentation and consultation requirements to support GRC 2. Provide on-site consultation services when requested. 3. Provide the following reports and documentation to the Agency: a. Quarterly drug reviews for each resident. Reviews identify drug interactions, drug side effects, and identify associated laboratory results related to medication use to determine safety and efficacy of treatment while working with GRC Interdisciplinary Team b. Report any irregularities in drug regimens to the prescribing physician and interdisciplinary team c. Provide an annual written report for each resident’s individual program plan
