Specifications include, but are not limited to: 1. The awarded vendor(s) shall create a report is to be done per unit that outlines the campus, area serviced (building, room, or space), date performed, type of system, manufacturer, model, serial number, system pressures, major component readings (rated and actual amps/voltage), belt size (if applicable), check list of components to be inspected, recommendations, repair costs and name of technician that performed the Preventive Maintenance Service (PM).; 2. Provide a minimum of 5 business days’ notice to LISA Academy Facilities team via email and copy the school admins to verify the campus has no events that would be affected by PM.; 3. The PM tech(s) are to check in to the front office and contact the admin team.; 4. The vendor is to provide the reports and repair quotes for every system within 5 business days of the campus PM completion via email and copy the Campus Facilities Team and the District Facilities Coordinator.; 5. Test blower components- Replace belts one’s year (1st PM) regardless of condition, if applicable. Units with extremely worn belts should be inspected for condition of sheaves and/or pulleys, along with the belt alignment and tightening THE BELT MUST BE NEW ORIGINAL EQUIPMENT MANUFACTURER(OEM); 6. The awarded vendor shall label all RTUs and its corresponding thermostats.; 7. Contractor must conduct all PM inspections within 4 weeks, if weather permitting and should designate at least 2 independent crews solely to this project during those 4 weeks.; 8. Fill out Preventive Maintenance Logs accordingly and issue them to the facility specialist or give access LISA Academy to get reports from their platform.; 9. The current ongoing service contract will be excluded from this contract for some campuses and those campuses will be informed later.; 10. All equipment, parts and/or supplies used must be new Original Equipment Manufacturer (OEM).; 11. The vendor must provide us the services as scheduled. Work shall be performed during normal working hours. Normal working hours are defined as 8:00 a.m. to 4:30 p.m. Monday through Friday including school breaks. At the discretion of LISA Academy, inspections may be conducted during State and Federal Holidays. A final schedule will be developed and agreed upon by the awarded vendor and LISA Academy.
