Specifications include, but are not limited to: Be a community behavioral health provider as identified in the Community Behavioral Health Provider document, attached as Attachment A, and must be located in North Dakota. If not a community behavioral health provider as identified in Attachment A, then describe what type of provider offeror is.; Serve at least one of the following populations: Adults with SMI; Children with SED; and/or; Individuals with SUD.; Utilize the funds as described and within the timeline identified in the Technical Proposal and Cost Proposal as approved by STATE.; Utilize funds in accordance with the Fund Utilization Examples, attached as Attachment B.; Make any necessary project modifications as deemed necessary by STATE.; Provide services in a manner that is recovery-oriented, trauma-informed, and person-centered.; Ensure retention and sign-on bonuses are not duplicated.; Coordinate with existing community behavioral health providers and programs to avoid duplication of services, as applicable.; Participate in training and technical assistance, including in-person and virtual meetings, provided by STATE.; Follow all federal funding requirements as outlined in the Federal Requirements, attached as Attachment C.; Submit monthly reports to STATE by the 10th of each month (unless otherwise directed by STATE), to include process and outcome measures, summary of project performance, implementation status, achievements, and barriers. A report template will be provided by STATE to the successful offeror(s).
