Specifications include, but are not limited to: NorthPoint is seeking a consultant to develop a patient flow strategic plan that will understand the current process of patient interface with the various co-located clinical areas including Medical, Dental, Behavioral Health, Human Services, and Health Information Management; identify ways to maximize efficient and effective client access to improved services; and create an implementation roadmap with timeline to create a positive experience for all NorthPoint patients. The plan should support patient and staff utilization of the new build out and expansion of the NorthPoint clinic to enhance integration that would improve services to NorthPoint patients. This work should address all aspects of participant/patient engagement as follows: • Multiple entry points to NorthPoint services (via virtual or physical) • Collaborate with existing contractors involved in current build out process (such as wayfinding) • Methods of appointment scheduling and accessing services • Appointment check-in process • Patient registration process/forms/HRSA data requirements (must address best process to request/collect needed information) • Interdepartmental navigation and communication of patient and clients
