Each firm that wishes to be considered as our hired benefits consultant are being asked to provide information on qualifications in writing about the items listed below, as well as any other information they wish to share about the services which they provide.; Lists of active municipalities/clients you represent with reference contact information. Copies of a sample “monthly claims report” provided to your customers – including: Medical/Rx Paid Claims Summary (Premium vs. Claims – for both medical and dental); Enrollment data year over year (medical and dental) including to-date averages; Incurred large claims tracking information – Claims over $50,000; Benefit Plan Performance Report – Loss Ratio by month and 12 month rolling average (medical only required).; Sample annual calendar of meetings, touch points, information reviews, decision deadlines, and other important information and planning dates.; A copy of who would be a part of the support team from your company – names, titles and their primary responsibilities.; All fees, commissions, over-rides, and compensation received from each employee benefit plan either annually or at the start of change of any existing plan.; Documentation of any and all ancillary services the City of Rhinelander may benefit from as a result of hiring you as our consultant.; Any other items you wish to share that may set you apart from other providers.
