Specifications include, but are not limited to: Contract Services must meet or exceed the mandatory requirements listed below. 4.1.1 Vendor Responsibilities 4.1.1.1 General Service Requirements: Vendor shall recruit, supervise, and compensate health care providers, pursuant to the terms of this contract, to meet the coverage needs of Lakin Hospital for the services set forth more specifically below. 4.1.1.2 Physician/Nurse Practitioner shall provide a maximum of one (1) day per month, which shall be determined upon mutual agreement of the parties, provide to the facility, through one (1) appropriately qualified Physician/Nurse Practitioner reasonably accepted to facility. The following services in accordance with currently accepted professional standards and applicable federal, state, and local laws and regulations while this agreement is in effect. 4.1.1.3 Physician/ Nurse Practitioner shall assist in identifying the gynecological needs of facility residents, evaluate and treat, to the extent clinically appropriate, residents’ gynecological conditions. All the above services shall be provided only on appropriate referral from resident’s attending physician and upon the consent of the resident or the resident’s representative. 4.1.1.4 Physician/Nurse Practitioner in accordance with facility’s policies and procedures, Provider shall document resident’s medical record of all services rendered to residents. 4.1.1.5 Continuity of Service: This contract is intended to provide continuity of Physician/Nurse Practitioner services and the management thereof on a continuous basis. In the event of termination of the contract by vendor, vendor must assume the continuity of health care services at a level consistent with the terms of the contract for a period not to exceed twelve (12) months from the notice of termination or until such time as DHHR can provide an alternate vendor.
