Specifications include, but are not limited to: The goal of the RHP program is to promote the health and well-being of refugees by: Providing opportunities for health literacy. Coordinating health care beyond RMS. STATE encourages collaboration with the local resettlement agency (RA), community and ethnic based organizations, educational institutions, and other service providers to complement health services. Successful offeror(s) shall not supplant or duplicate existing care coordination and health literacy services funded under federal and ORR funding sources including RMS, Medicaid, ORR’s Preferred Communities (PC) Program, Reception and Placement (R&P), etc. The successful offeror(s) must follow these guidelines when implementing the RHP programming: Provide opportunities for health literacy. Provide opportunities to increase health literacy and empower clients to make informed health decisions. During this process, the successful offeror(s) shall: Have a clear strategy for delivering appropriate health topics and selecting the target population, including deciding on group classes versus individual outreach. Provide user-tested or validated curricula and materials, if possible. Provide classes in a culturally and linguistically appropriate manner. Conduct pre- and post-assessments to gauge progress. For group activities: Regularly assess relevant health topics, activities, and identify target populations. Provide services based on validated curricula, if available, effective interventions, or promising practices.
