Specifications include, but are not limited to: The Indiana Family and Social Services Administration (FSSA) manages the [MLTSS Program Name] program, which will serve approximately 106,000 Hoosiers. [MLTSS Program Name] is a statewide coordinated care program for Indiana’s Medicaid enrollees who are 60 years of age and older and are eligible for Medicaid on the basis of age, blindness, or disability and have limited income and resources. All individuals who are 60 years of age or older in the target eligibility categories will be included unless they meet an exclusion, as specified in Exhibit 3. Enrollees include members who have a full Medicare benefit, those in a nursing facility, and those who are receiving long-term services and supports (LTSS) in a home or community-based setting. The State is interested in contracting with MCEs that can perform the administrative functions of a typical insurer, can support members in strategies to address complex and chronic health conditions of the program population which includes many individuals with complex health conditions who are dually eligible for Medicare and Medicaid and/or receive LTSS, and can manage and integrate care along the continuum and settings of LTSS in Indiana.
