Specifications include, but are not limited to: A. The Contractor must recruit and provide guidance to support the implementation of new ADArecognized/ADCES-accredited DSMES programs in Idaho community and healthcare organizations not contracted with the Department. 1. Provide guidance to the organization on whether to pursue ADA-recognition or ADCESaccreditation. 2. Utilize existing and current diabetes data sources to drive evidence-based support to include, but are not limited to the following: a. DSMES Toolkit (https://www.cdc.gov/diabetes/dsmes-toolkit/index.html). b. 2022 National Standards for DSMES (https://diabetesjournals.org/care/article/45/2/484/140905/2022-National-Standardsfor-Diabetes-Self). c. ADCES 2022 National Standards for DSMES Interpretive Guidance (https://www.diabeteseducator.org/docs/default-source/default-documentlibrary/2022-interpretiveguidancee252f836a05f68739c53ff0000b8561d.pdf?sfvrsn=bd0d9058_0). d. ADA 2022 National Standards for DSMES Interpretive Guidance & Checklist (https://professional.diabetes.org/files/media/2022-dsmes-standards-interpretiveguidance-final-3-4-22.pdf). 3. Provide a DSMES Implementation Report quarterly. 4. Provide a copy of ADA-recognition or ADCES-accreditation certificate once the certificate becomes available. B. The Contractor must promote and facilitate statewide quarterly learning collaboratives for Idaho DPP coordinators, lifestyle coaches, and data preparers. 1. Topics must include, but are not limited to: a. Program establishment and implementation. b. Program maintenance and management. c. Participant engagement and retention. d. Program reimbursement and sustainability. e. Medicare National DPP (MDPP) expansion. 2. Provide a Quarterly Learning Collaboratives Report. C. The Contractor must recruit and provide guidance to support the implementation of new CDCrecognized National DPPs in Idaho community and healthcare organizations not contracted with the Department.
