Specifications include, but are not limited to: Proposals to develop and implement a pilot program of falls prevention using the CAPABLE program for the purposes of assessment, intervention and falls prevention, including working with Choices for Care and Money Follows the Person participants, must demonstrate the following capabilities: 1. Demonstrated dedicated staff capacity and/or ability to recruit and retain dedicated staff necessary to carry out contract functions. 2. Ability to pilot CAPABLE, working with support from CAPABLE staff in Baltimore to ensure fidelity to CAPABLE protocol. 3. Ability to assemble the necessary contracted staff/personnel to provide RN, OT and home modification expertise to carry out CAPABLE program and to ensure they receive the mandatory CAPABLE training. 4. Demonstrated experience with the needs of, and barriers faced by, older Vermonters and Adults with disabilities who wish to remain safely in community-based settings of their choice. 5. Demonstrated collaborative relationships with organizations that provide case management services to older Vermonters and adults with disabilities, including those individuals enrolled in Choices for Care and Money Follows the Person.
