Specifications include, but are not limited to: Community-Based Housing Support on behalf of the Health and Human Services Housing Stability Area.; Long-Term Homeless Supportive Housing: The LTH program houses individuals who have experienced long periods of homelessness and expands tenant-based permanent supportive housing opportunities in integrated housing. Tenant-based rent subsidies and housing support services follow clients over time and across housing choices, providing maximum financial and emotional continuity. This approach minimizes the demands on clients to form new relationships with different service providers if the client graduates or fails at a specific or site-based program. It is consistent with Olmstead v. LC, 119 S. Ct. 2176 (1999), which requires maximizing integration.; LTH providers must participate and comply with all requirements of the HUD (Housing and Urban Development) coordinated entry system (CES) and HMIS (Homeless Management Information System) reporting requirements. Hennepin County has additional standards and requirements beyond the State, including fidelity standards. Proposers will be held accountable to serve the target population for which they have proposed. Housing support agreements will be contingent upon serving the proposed target population.; Housing with Services-Independent: HWS-I opens Housing Support to individuals with a disability and seniors who want to live in a more independent housing setting, such as their own apartment, without needing to be long-term homeless. The individuals certainly may be long-term homeless and that is a high priority target population.; • Base-rate housing support (GRH) only; services may be provided with Medicaid, Medicaid waivers, DHS HSASMI grant, or other service funding by a qualified provider.; • No LTH or CES requirement for individuals or buildings.; • Buildings are required to have a habitability inspection.; • Housing requires a lease and tenant-landlord law applies. The service provider may master-lease, but the provider also must have a fully legal lease with the tenant and transition the individual into their own independent lease.; • For waiver service participants under a master-lease, it is required that a plan is developed that addresses the transition of the master-lease into the participants name within 2 years.
