Specifications include, but are not limited to: Provide direct support and consultation (via phone and email) to professionals and families of children and youth with special heath care needs · Provide or coordinate access to trainings for families and professionals in the field of special needs (e.g., teachers, case managers, clinical staff. etc) on resources and services for children and youth with special health care needs, medical home, and youth transition concepts · Assure links to other valuable resources, cross-refer between programs, identify common unmet needs, and strengthen the existing system of care for children and youth with special health care needs and their families · Conduct targeted outreach to ensure awareness of services and to build positive relationships with other local and/or regional programs · Seek out underserved communities and identify strategies to increase awareness of children and youth with special health care needs resources and support · Ensure family, youth, and community voices are included in program planning and improvement activities
