Specifications include, but are not limited to: Supportive Services staffing to support 12 individuals including program management, 18case manager/housing navigator, and peer support positions, etc. as needed to meet thescope of work.Conduct problem-solving conversations and/or coordinate with diversion/rapid resolutionservices to connect the household to other suitable options outside of the homelessservice system.Provide access to the Coordinated Entry System (CES) for Lane County for householdsstaying in shelter, including acting as Front Door Assessor for population served,attempting assessments for 100% of eligible households staying at least 14 nights.Staff are expected to work with each individual to develop an individualized housingstability plan that outlines action steps that will be taken to achieve housing stability.Individuals are expected to transition to permanent housing options in less than 120days whenever possible, however there is no specific limitation on length of stay.Seedetail in Length of Stay below.
