Specifications include, but are not limited to: furnishing and installing (except as noted) all materials labor and equipment necessary to resurface or rehabilitate streets or roads as well as reconstruct existing sidewalks as listed, including: utility cover adjustments: water gate box lowering and raising, tack coat, shim, and overlay; driveway preparation and lawn and driveway restoration; pavement cold planing, pavement removal; underdrain, culverts, storm drain, catch basins, manholes, clearing, ditching and riprap; common excavation, aggregate base gravel; erosion and traffic control and cleanup.; River Street, from Cedar Street to Lincoln Street: Scope: SIDEWALKS: Remove all pavement from existing sidewalks. Box cut 12” and remove existing curb. Add 12” (9” of MDOT Type D & 3” of MDOT Type A) of Gravel, replacing existing curb with new slip-form concrete curb. Ensure all curb ramps meet ADA Standards and install detectible warning plates. Move sidewalk in 10’ to create 5 new parking spaces on River Street along Lionel Potvin Park, box cutting 18” and adding 18” (15” of MDOT Type D & 3” of MDOT Type A) of gravel in the new parking area.. Pave all remaining sidewalk areas with 2” of 9.5 mm HMA. Perform lawn repair as needed.; ROADWAY: Lower and plate all manholes and water gates to prep for pavement removal as needed.. Remove pavement from entire roadway. Place 2” of 19.0 mm HMA across entire roadway. Adjust structures to final grade. Prep driveway entrances. Pave 1.5” of 12.5 mm HMA Surface. Crown must be maintained within the roadway. Perform driveway apron paving as necessary at a depth of 2”.; Oxford Street, from Cedar Street to River Street: Scope: SIDEWALKS: Remove all pavement from existing sidewalks. Perform lawn repair as needed.; Each bidder is required to state, in his/her proposal, his/her name and place of residence and the names of all persons or parties interested as principals with him/her, and that the proposal is made without any connection with any other bidder making any proposal for the same work, and that no person acting for or employed by the City of Lewiston is directly or indirectly interested in the proposal or in any contract which may be entered into to which the proposal relates, or in any portion of the profits there from, except as provided by the City Charter.; The Contractor shall give his/her personal attention constantly to the faithful prosecution of the work, shall keep the same under his/her personal control and shall not assign, by power of attorney or otherwise, nor sublet the work or any part thereof, without the previous written consent of the Purchasing Agent; and shall not, either legally or equitably, assign any of the money payable under this Agreement or his/her claim thereto, unless by and with the like consent of the Purchasing Agent. Proof of insurance coverage will also be required from the subcontractor and any liabilities on the part of the subcontractor will be the responsibility of the contractor under his contract with the City and the City will be held harmless. The contractor shall take all necessary affirmative action steps to assure that qualified minority businesses, women’s businesses, and labor surplus area firms are included in the hiring of any subcontractor working on this City project.
