Specifications include, but are not limited to: 1) Protective footwear shall be hard-toe, hard-soled, boots that cover the ankle (minimum 5-inch height) and meet the American Society for Testing Materials (ASTM) F2413 requirements. Approval must be found on the boot. Specifically excluded are tennis, fabric, or similar shoes.; Additional recommended features for work boots include: • Lace-up ; • A defined heel ; • Slip resistant soles; • Rugged tread ; 2) On a separate sheet, vendor(s) shall explain how the voucher program will work and provide a copy of an approved paper or electronic voucher that will be used when purchasing the safety footwear in their bid response. ; 3) On a separate sheet, vendor(s) shall provide a list of approved stores, addresses (including city), phone (including area code) and store hours in their bid response. ; 4) On a separate sheet, vendor(s) shall provide a list of safety footwear that meets the ASTM standards for INDOT’s review, approval and inclusion into the safety footwear program. ; 5) On a separate sheet, vendor(s) shall explain their standard return policy.
