Specifications include, but are not limited to: A purchasing agreement with a professional or company that can provide in home participant Accessibility/Mobility Assessments which includes individual home modification recommendations. In addition, the contractor would provide consultation with the state and state contractors as appropriate. The Contractor will be expected to: 1. Schedule, coordinate and complete an estimated one hundred (100) Accessibility/Mobility Assessments for DDSD/ASD funded local service provider agencies throughout Vermont during the one-year pilot program. 2. Contact a provider agency within five (5) working days of receiving a request for an Accessibility/Mobility Assessment to schedule the assessment. The Accessibility/Mobility Assessment will be completed within ten (10) working days of initial contact, unless the Contractor and the local service provider agency requesting the inspection agree to other arrangements in writing. Contractor will provide a copy of this written agreement to the State, if requested, when submitting the invoice for the assessment. 3. Perform Accessibility/Mobility Assessments as agreed upon between the state and the contractor. Accessibility/Mobility Assessments will include the evaluation of all areas of accessibility and mobility in the residence including, but not limited to, entrances and exits, (walkways, handrails, doorway widths, ramps), common areas, bathroom(s), and entrance to and egress from the home. When conducting the assessment, the Contractor will consider the needs of the individual receiving DAIL-funded services and ensure the individual’s needs are assessed.
