Specifications include, but are not limited to: Measurable Intended Outcomes 1. Expanded state health department capacity to prevent and control COVID-19 infection (or transmission) among populations at higher risk and that are underserved, including racial and ethnic minority groups and people living in rural communities by utilizing community-based organizations as a bridge between communities with the highest social vulnerability and resources to address their social determinants of health. 2. Reduced health disparities and inequities in DE communities, emphasizing those with the highest Social Vulnerability Index (SVI). 3. Improved social determinants of health among populations at higher risk and that are underserved, including racial and ethnic minority groups and people living in rural communities.
