Specifications include, but are not limited to: Attend and participate in DDC Advocacy and Outreach Committee meetings as well as Council meetings as requested to gain input and guidance from the committee/council and provide updates on the progress of the project.; Contractor will reach out to potential guest speakers to secure them for the event. This will include outreach via email, mail and phone as needed. Potential guest speakers are as follows: Jonathan Martinis, Esq.; Alexandra McFassel with the Office of Public Guardian; a representative from the Disabilities Law Program; Melissa Bryson with Delaware ACLU and Jennie Hatch. Contact information can be provided by DDC staff if needed. Approval must be obtained from Social Services Administrator, Executive Director, and DDC’s Advocacy and Outreach committee before proceeding with any speaker agreements.; Contractor will work the following agencies; previous Summit partners, to identify current alternative options available to families. These included the Department of Health & Social Services (DHSS) including their Division of Developmental Disabilities Services (DDDS), the Division of Services for Aging and Adults with Physical Disabilities (DSAAPD), and the Division of Substance Abuse and Mental Health (DSAMH), the Disabilities Law Program Community Legal Aid, Inc. (DLP), the Office of the Public Guardian (OPG), the Governor’s Advisory Council for Exceptional Citizens (GACEC), the State Council for Persons with Disabilities (SCPD), and persons with disabilities and family members.; Contractor will outreach to other organizations in Delaware to assist with sharing information about the event including but not limited to the Medical Society of Delaware and the Department of Education’s Transition Cadre team.; A major activity of this project is to focus on increasing the levels of awareness of SDM through use of educational experiences that will be tailored to older adults and people with I/DD at times of transition when guardianship decisions are common (e.g., transition from the Department of Education (DOE) services to adult services). In addition, educational experiences will be tailored to increase the levels of awareness of SDM in other targeted populations including: those directly involved in a person’s SDM; those providing supportive services; professionals and others providing care; legislators and the general public. ; With input from the DDC Community Relations Officer, Social Services Administrator and Executive Director, the Contractor will develop targeted marketing strategies and materials that DDC’s Community Relations Officer will share on the DDC Social media pages. This event will rely heavily on boosted posts to increase awareness and attendance to the event. This information will also be shared with the National Association on Council with Developmental Disabilities (NACDD) to share with the national list serve.; Contractor will secure translators and reasonable accommodations as requested by participants who register for the event. Contractor will ensure adequate funding is set aside within the budget to secure these accommodations as needed.
