Medical Cards color is solid blue with screened percentages to make the color appear differently. There are continuous pin feed holes on the edges of the form to feed the forms through the printer. The size of each individual form before being processed through the seal is 8 ½” X 11”. Medical Cards must be provided as displayed in attachment to this solicitation labeled Medical Card Example and Medical Card Example 2. The “x” on the examples must be deleted. Card needs to be able to be torn out.
