Specifications include, but are not limited to: The Metropolitan Health District is seeking the professional services of a Licensed clinical social worker (LCSW) – Master’s degree and State license to serve as a Teen Health Navigator. In doing so, the City seeks Respondents to propose the following: Serve an administrative role in the BASE education social-emotional online curriculum, which includes monitoring and respond to participants needs as they arise including follow-up with school counselors, parents, and referral to community support services. Provide support services for Teen Ambassadors (program participants) who exhibit or express a need for support services. Support services may include up to 2 online or telephonic counseling sessions and referral and navigation to community counseling services if needed. Maintain a counseling caseload of no more than 5 program participants at any given time.
