Sample folder will be provided to Contractor; Front cover, back cover and a middle divider with a 5/16” tab approx 5-3/8” long in the middle of the divider. A permanently affixed tab is attached on the back cover. 1/3” cut metal tab in 1st position on the 11-3/4” side. Tab must be 4” (w)” attached with metal type eyelets.
