Specifications include, but are not limited to: 2.5.1 Contractor shall provide printed reports upon request within 24 hours of spray application.; 2.5.2 Contractor shall provide a written emergency response action plan identifying action steps in the event of an emergency/accident or a chemical release/dump.; 2.5.3 Contractor shall be responsible for all licenses, permits, fees, and all other items required of aerial applicators of pesticides.; 2.5.4 The chemical to be sprayed shall be Permethrin with Piperonyl Butoxide as the synergizer.; 2.5.5 Contractor shall be responsible for providing chemical.; 2.5.6 The application rate for Permethrin shall be .007 lbs. of active ingredient per acre.; 2.5.7 Contractor shall comply with all label directions for aerial application and the adulticide shall be applied according to manufacturers' label, terms and specifications.; 2.5.8 Contractor shall comply with all applicable statutes, ordinances, rules, regulations, and orders of all public agencies and authorities relating to the aerial application of pesticides over residential areas in the State of North Dakota; 2.5.9 The selected contractor will assume sole responsibility for staff and subcontracts under the agreement and serve as the sole point of contact for contractual matters including payment.; 2.5.10 Subcontractors are subject to City approval.; 2.5.11 The selected contractor is responsible for providing approval from the North Dakota Department of Health and the F.A.A. prior to aerial applications.; 2.5.12 The selected contractor is responsible for notifying beekeepers in the area and could be liable for damages to beehive populations resulting from an aerial spray for mosquitoes.
