Specifications include, but are not limited to: 1. Waste Transporters Identify all waste transporters that will be used during the performance of the contract. Provide the name, address, phone number, and EPA identification number of each transporter. All waste transporters must have any and all federal, state and local permits required for the transport of hazardous materials and/or waste. After contract award, any changes or substitutions in the designated transporters must be pre-approved in writing by ISDA.; 2. Transfer Storage and Disposal Facilities (TSDFs) 2.1. The Offeror must identify by name, address, and EPA/DEQ Identification number, all TSDFs that will be used during the performance of the Contract with their proposal. Any changes or substitutions in TSDFs after contract award must be pre-approved in writing by ISDA.; 2.1.1. Include a brief compliance history for each facility. This must include a description of any violations assessed under the Resource Conservation and Recovery Act (RCRA) during the past five (5) years along with a description of any remedial or corrective actions currently being investigated or conducted under RCRA or the Comprehensive Environmental Response, Compensation, and Liability Act of 1980 (CERCLA). Failure to fully disclose information in your response, to the best of your knowledge, may result in your Proposal being found non-responsive or may be grounds for Contract termination if you are awarded a Contract and the omission is discovered after Contract award.; 2.2. If any of the proposed TSDFs has been audited by the Offeror for compliance under RCRA within the last two (2) years, provide the date(s) of the audits and the dates of any available audit reports. (Note: Audit reports should not be included with the Proposal; however, ISDA reserves the right to review audit reports upon request.); 2.3. Indicate if the proposed TSDFs have previously received or managed any waste that was considered to have been generated by any state governments.; 3. Collection Management – The Contractor must complete each collection as a separate event. For each collection, the Contractor must: 3.1 Complete the collection in one day, while conforming to all federal and state regulations.; 3.2 Remove all waste from the collection site within twenty-four (24) hours of the end of the collection day. ; 3.3 On collection day, arrive at least one hour prior to collection event start time, set up the site at least ½ hour prior to collection event start time, and immediately address spills and leaks within the HASP guidelines.
