Specifications include, but are not limited to: proposals for Care Coordination services in order to assist Family Caregivers of individuals with dementia or individuals ages sixty (60) years and older with complex health needs receiving care remain in their homes as independently as possible in safety and with dignity. Qualified vendors must have: • Working knowledge of community-based supports and services. • Knowledge of effective marketing and outreach practices. • The ability to coordinate services. • An understanding of the Family Caregiver role in the coordination of services 3.2.1. The selected Vendor must establish community-based linkages in order to build a repository of resources that are available to Family Caregivers. 3.2.2. The selected Vendor must work with Family Caregivers to ensure each individual for whom care is provided, has a plan of care that provides a roadmap for integrating care based on the individual’s or family’s needs and priorities. 3.2.3. The selected Vendor must ensure the family-centered roadmap includes, but is not limited to: 3.2.3.1. Health summary. 3.2.3.2. Long-and short-term goals – which are specific, measureable, achievable, relevant, and time specific (SMART). 3.2.3.3. Upcoming appointments. 3.2.3.4. Emergency and disaster plans.
