Specifications include, but are not limited to: A. Consultation with the Diabetes, Heart Disease, and Stroke Prevention (DHDSP) Program – The Contractor must: 1. Participate in up to three (3) initial meetings with the Department to discuss project scope, survey design, development, and dissemination.; 2. Provide feedback on a draft survey developed by the Department and make recommendations on question design, placement, and logic.; 3. Develop and maintain a Project Management Plan, including project timeline, identifying key milestones, roles and responsibilities.; a. This plan must be provided to the Department within thirty (30) calendar days of the Contract’s execution. ; B. Survey Dissemination – The Contractor must: 1. Work in collaboration with established partners (state associations, alliances, public health districts etc.) to disseminate the survey to Idaho health systems. A list of statewide associations will be provided by the Department.; 2. Pilot the survey with up to five (5) health systems or at the discretion of the Department prior to releasing the survey statewide.; 3. Disseminate the survey electronically to Idaho health systems. ; C. Data Analysis – The Contractor must: 1. Conduct preliminary coding and data cleaning.; 2. Conduct descriptive and inferential statistics on survey data to answer questions developed by the DHDSP Program. ; D. Reporting – The Contractor must: 1. Produce a final report and an interactive Tableau dashboard or similar for data visualization of results.; 2. Provide up to two (2) virtual presentations on the results of the survey to stakeholders. a. Stakeholder presentations will be organized by the Department.
