Specifications include, but are not limited to: The State of Delaware, Department of Health and Social Services (DHSS), Division of Public Health (DPH), is seeking proposals from a potential vendor(s) to establish and operate two new high-needs elementary school-based health centers. A vendor can choose to submit a proposal for one or a combined proposal for both schools. DPH will provide the following toward the accomplishment of this project: a) Base formula funding to the selected medical vendor to operate the SBHC. DPH reserves the right to revise the formula based on availability of funding. (for medical vendors only) b) One time funding to sustain continued operations of an existing SBHC, and/or expand an existing high need elementary SBHC.(for medical vendors only) All application proposals must contain a strong justification for one-time funds being requested and must show the costs are reasonable and necessary to carry out the program’s purposes. c) Capital funds for $250,000 minimum for high needs elementary SBHCs, Lead Education Agencies (LEA), School Districts/Charter Schools
