Specifications include, but are not limited to: This Request for Applications (RFA) is published to solicit applications for services to increase the wellbeing and health of New Hampshire citizens by preventing and reducing homelessness, and increasing successful placements to permanent housing. Services are funded by the Emergency Solutions Grant (ESG). Vendors may apply to provide services for any or all of the following program components: Street Outreach Rapid Re-Housing Homelessness Prevention Housing Stabilization The Department of Health and Human Services (Department), Bureau of Housing Supports (BHS) anticipates awarding up to nine (9) contracts to different vendors across the state, with the goal of having services available in all geographical regions, statewide as indicated below. Belknap/ Merrimack Counties Strafford County Cheshire/ Sullivan Counties Coos/Carroll/ Grafton Counties Rockingham/ Hillsborough Counties excluding the towns listed for Greater Nashua as indicated below. Greater Nashua (City of Nashua and towns of Amherst, Brookline, Hollis, Hudson, Litchfield, Mason, Merrimack, Milford, and Mont Vernon) ESG is a United States Department of Housing and Urban Development (HUD) formula grant program. Eligible recipients are states, metropolitan cities, urban counties and territories that apply through the Consolidated Planning process. As the HUD State Recipient for ESG, the BHS must subgrant all ESG funds to units of local government and/or to private, nonprofit organizations. Distribution of funds will be achieved through this competitive procurement process. The ESG program provides funding to: Engage individuals and families experiencing homelessness; Rapidly re-house individuals and families experiencing homelessness; and Prevent families and individuals from becoming homeless.
