Specifications include, but are not limited to: Create and implement Suicide Prevention evaluation plans, in alignment with the requirements of federal funding sources. Develop and implement an evaluation plan for Suicide Prevention interventions targeting individuals between the ages of eleven (11) and twenty-four (24); and Develop and implement an evaluation plan for Lifespan Suicide Prevention interventions targeting disproportionately impacted populations. Develop and report on Performance Measures Work with organizations designated by the Department and the Department to develop a Performance Measure Monitoring Plan for relevant Performance Measures; Develop tools that will allow participating organizations to collect Performance Measure data; Develop a tracking system for collating Performance Measures from participating organizations; Provide technical assistance, as needed, to participating organizations when collecting Performance Measures data; and Develop and submit an annual progress report of activities under the Performance Monitoring and Evaluation Plan. Collect data and document the evaluation process. Synthesize, analyze, and interpret Findings. Present recommendations to the Department on how to leverage Findings to ensure improvements to the Department’s related programs. Report and disseminate Findings to various Department partners including, but not limited to: Department leadership and applicable staff; Other State departments and agencies as determined by the Department; Federal grant funders; and Community Suicide Prevention partners and other stakeholder groups.
