Inside to be printed 1/1 black ink throughout on white 20lb recycled bond, grayscale photo on the Welcome to MaineCare page. Electronic copy supplied as MS Word document (see attached). Vendor will need to convert file if needed. Electronic and Hard copy proof required. Hard copy proof must be delivered to agency (Attn: Shannon Martin, 6 Alder Lane, Belgrade, ME 04917). Overrun NOT TO EXCEED 3%. Price is per each book. Packaging: Boxed in 50 or 100's. Uniform packaging required. Completed job must be delivered within two weeks (10 working days) of proof approval. NOTE: Delivery location is DHHS, MaineCare Services 109 Capital Street, Augusta, ME 04330. There is NO loading dock at this location. Truck with liftgate will be required for pallets. Inside Delivery Required. EXTENSION: The State of Maine reserves the right to extend the purchase order for up to five years from the date of the original purchase order for reprints. Total period may not exceed 5 years. Note: The same quantities must be ordered, and vendor must hold price in order for extension to apply. RECYCLED CONTENT: Bids offering paper or paper products with a minimum of 30% PC recycled material content that are within 10% of the lowest bid that meets all other specifications may receive up to a 10% price preference. In order to be considered, you must indicate that you are bidding recycled paper. If bidding recycled, please disclose the recycled content in the comments section of your solicitation response.
