Specifications include, but are not limited to: The Department of Health and Human Services (Department) is seeking a National Diabetes Prevention Online Delivery Platform as defined in this Request for Proposal (RFP) document. This document provides instructions for submitting proposals, the procedure and criteria by which the awarded Bidder will be selected, and the contractual terms which will govern the relationship between the State of Maine (State) and the awarded Bidder. The Department is dedicated to promoting health, safety, resiliency, and opportunity to all Maine Residents. The Maine Center for Disease Control and Prevention (Maine CDC) provides the leadership, expertise, information, and tools to assure conditions in which all Maine people can be healthy. Maine CDC’s Diabetes Prevention and Control Program (DPCP) was initiated in 1977 and continues to promote excellence in Diabetes prevention and control, to increase access to prevention and care, and to support and promote efficient and effective health care. The purpose of this RFP is to procure an online/virtual platform that facilitates the delivery of National Diabetes Prevention Program (National DPP) by local Lifestyle Coaches. This online/virtual platform will allow local programs to meet all levels of recognition and include the collection and export of deidentified data. These online/virtual platform services are expected to allow local National DPP’s to expand their National DPP services to a much larger population to those who are at risk of developing and/or diagnosed with prediabetes. Specifically, the online/virtual platform shall reach individuals who are not able to attend in person National DPP services.
