Specifications include, but are not limited to: Contractor shall provide Underground Fuel Tank Inspection, Training and Repair Services at locations specified in Exhibit B-1, UST Location and Equipment Listing. Contractor will be responsible for providing California Unified Program Agencies (CUPAs) with information regarding designated UST Operator for each office at the beginning of the Agreement term (see example, Attachment 1*). Contractor to perform UST Designated Operator (DO) functions for the California Highway Patrol in accordance with California Code of Regulations, Title 23. Functions include the following: 1. Contractor shall provide a list of employees, broken down by offices, each will visit; a schedule of monthly visits and documentation that each office was inspected, and the appropriate CHP employees trained. All documentation will be submitted to the CHP Project Representative. 2. If Contractor certification expires or if there are any changes to personnel within the term of this agreement, Contractor must provide CHP with a new owner’s statement 60 days prior to change. Contractor must also notify CUPAs no later than 30 days after the change.
