Specifications include, but are not limited to: Aid brain injury survivors and their families in identifying and obtaining the necessary supports to live productive and meaningful lives within the community. Provide Statewide information and referral services through a telephone hotline. Hotline shall, at a minimum, operate between the hours of 8:00 a.m. and 4:45 p.m. EST. Provide a monthly report of any downtime which occurred during operating hours for the previous month. Provide community resource coordination (outreach and information) to hospitalized Acquired Brain Injury (ABI) survivors including: Written and electronic material describing supports and services available for survivors post-hospitalization; Contact information for supports and services; and Warm handoff to a community provider as part of discharge planning in coordination with the hospital.
