Specifications include, but are not limited to: The Health Transformation Specialist will assist state staff with managing the CHART model implementation, which includes: • Collaborating with state staff and participating hospitals. • Organizing and helping run advisory council meetings. • Recruiting additional participating hospitals. • Recruiting aligned payers. • Assisting with developing and monitoring CHART budgets and expenditures. • Overseeing and assisting with the work completed by the finance contractor. • Assisting with developing and writing CHART related state plan amendments, administrative rule changes, and accompanying notice documents as well as assisting the state with the approval process. • Assisting with developing and writing policies related to the CHART model and the health transformation plan.
