Specifications include, but are not limited to: The expansion of volunteer programs and activities within the Area Agency on Aging system for the purposes of supporting Choices for Care and MFP participants to transition from Skilled Nursing Facilities to community-based settings and/or for those individuals to safely remain in community-based settings by enhancing unpaid caregiver support must demonstrate the following capabilities: 1. Demonstrated dedicated staff capacity and/or ability to recruit and retain dedicated staff necessary to carry out contract functions. 2. Demonstrated experience in creating and supporting volunteer programs serving older Vermonters and individuals with disabilities. 3. Proven ability to create, support, and modify trainings and training materials in the areas of volunteer management, companionship and respite services, creative problem solving and combatting social isolation/loneliness. 4. Collaborative relationships with State and community partners who can provide input on trainings related to Assistive Technology, community connections, mental health, stress reduction, and self-care. 5. Administrative and case management capacity to carry out work specifications.
