The items listed below are services DHDSP is looking to implement: Consultation with the DHDSP Program • Participate in up to three (3) initial meetings with the Department to discuss project scope, survey design, development, and dissemination. • Develop a draft survey and address and incorporate no more than two (2) rounds of feedback provided by the DHDSP Program. Make recommendations on question design, placement, and logic. • Develop and maintain a Project Management Plan, including project timeline, identifying key milestones, and roles and responsibilities. • Meet with the Department monthly, following final survey approval, at a date/time specified by the Department, to report out on progress.
