Specifications include, but are not limited to: Health Literacy Programming: Provide opportunities to increase health literacy for eligible populations and empower clients to make informed health decisions. The successful offeror shall have a clear strategy for delivering appropriate health topics and selecting the target population. The successful offeror should utilize a user-tested or validated curriculum and materials, if possible. The successful offeror should provide classes in a culturally and linguistically-appropriate manner and should conduct pre and post-assessments to gauge the level of knowledge gained by recipients. Access to medical, emotional wellness services, and affordable health care: Provide services to ensure individuals most at-risk can navigate and access complex health care systems. Within this framework, the RHP program aims to build capacity to address identified health needs within refugee communities and reduce barriers to achieving wellness. A plan should be developed to create peer support groups within cultural communities that provides bonding opportunities, addresses trauma and hardships, and overcomes obstacles that are common to refugee populations. Group leaders should be trained to connect individuals with the next level of services should they need it. Disease prevention and health promotion activities: The program is designed to coordinate and promote physical and mental health services, including disease prevention strategies. The RHP funds are intended to enhance access to health care services through engagement and partnerships among health care facilities, services, and community-based organizations in a manner that complements existing care coordination.
