The services provided under this RFP are intended to: Increase Patient Care Coordination secure information sharing; Improve Patient Medication Management safety; and Gain efficiency and increase accuracy of Patient Billing and Financial Services. As part of this modernization effort, the Department intends to implement a suite of ICM Solutions to support more effective care coordination, planning, and delivery decision-making than is possible with the Current Legacy Systems. This RFP address some but not all the ICM Solutions described in Exhibit 1 Legacy Systems, Exhibit 2 RFP Scope for ICM Solutions, and Exhibit 3 Conceptual Diagram. In partnership with the Department’s Medicaid Management Information System (MMIS) program, the awarded Bidder shall implement the proposed ICM Solutions, replacing single purpose systems with fully integrated solutions that manage patient information across pharmacy, clinical, medical records and financial teams and other State staff supporting patient care. The modernization effort shall improve internal efficiency across solutions and implement interoperability in the context of the MMIS claims payment solution, Prescription Drug Monitoring Program (PDMP) and the planned Patient Information Access and Exchange Solution which shall build upon the existing statewide Health Information Exchange (HIE). The suite of ICM Solutions outlined in this RFP shall provide the ability to enforce plan of care limits, audit trails, eliminate redundant data entry, enhance integration with other key systems, support longitudinal reporting capabilities, and enhanced technical and business documentation to improve training and supportability.
