Specifications include, but are not limited to:. Program Support Requirements Develop and manage a Memorandum of Understanding (MOU) or Memorandum of Agreement (MOA) for each partner Federally Qualified Health Center (FQHC), prior to work beginning with the respective FQHC. The MOUs/MOAs shall clearly define the goals of the partnership, activities that will be conducted (including data requirements), and the responsibilities of each partner FQHC. Develop a Technical Assistance (TA) Plan that describes how implementation support will be provided to the partner FQHCs for data system improvements, evidence-based interventions (EBI) implementation, data monitoring, and reporting for each Program Area. Provide data collection from the partner FQHCs including the use of a universal data system(s), including Community Health Center Network of Maine (CHCNet).
