Specifications include, but are not limited to: The Department of Health and Human Services (Department) is seeking evaluation of multiple Maine Center for Disease Control and Prevention (Maine CDC) Public Health Programs, as defined in this Request for Proposals (RFP) document. This document provides instructions for submitting proposals, the procedure and criteria by which the awarded Bidder will be selected and the contractual terms which will govern the relationship between the State of Maine (State) and the awarded Bidder. The Department is dedicated to promoting health, safety, resiliency, and opportunity to all Maine Residents. The Maine CDC provides leadership, expertise, information, and tools to assure all Maine residents have the access and opportunity to live healthy lives. The goal of the Maine CDC’s Chronic Disease Prevention and Control Program is to implement evidence-based programs for chronic disease prevention, by combining environmental approaches, collaborating with health care systems, and developing clinical and community linkages. The Chronic Disease Prevention and Control Program develops and applies collaborative strategies to improve the health of Maine residents by: Mobilizing partners to support evidence-based prevention efforts; Gathering and disseminating data to promote healthy decision making; Empowering patients to self-manage chronic disease; Promoting evidence-based guidelines to strengthen health care systems; Linking health care systems with community resources; Increasing access to screening of early detectable chronic diseases; and Promoting evidence-based screening recommendations.
