Specifications include, but are not limited to: At a minimum, the scope of services required by the successful firm(s) shall consist of the following from qualified individuals or firms, the following services for Select plan members and their enrolled dependents at no cost to those members: A minimum of three Select Clinic locations with flexible hours and days of operation Primary Care Services, including but not limited to, preventive care services such as annual physicals, diabetes screening and education, blood pressure and cholesterol screenings, pediatric services, men’s health, gynecological care and health information, and women’s health services. Also, the ability to diagnose and treat allergies, infections, cold, flu, cuts, bruises, strains, sprains and offer free COVIID-19 testing Voluntary family planning services, well-child services, prenatal and perinatal services, screening for cancer and communicable diseases Urgent Care Services including, but not limited to, upper respiratory conditions, bronchitis, urinary tract infections, viral illnesses, minor trauma The chosen vendor(s) will need to perform basic mental health services/screenings of all patients at each clinic visit Vendor will offer vaccinations including influenza, COVIID-19, tetanus, Hepatitis A/B, and other immunization service The chosen vendor(s) will need to provide access to Electronic Medical Records (EMR) that provide patients with the ability to share detailed clinic visit usage and outcomes with their designated primary care physician Chosen vendor(s) will need to have resources in place to refer members back to their primary care physician (currently Kelsey-Seybold) for follow-up care, as needed Chosen vendor(s) will need to understand the District’s pharmacy benefit manager’s formulary to ensure they are writing prescriptions, when needed, that are cost efficient to patients Chosen vendor(s) will need to display the ability to diagnose members with more acute care needs and help educate them on the importance of following treatment plans created in collaboration with their primary care physician. Chosen vendor(s) will need to work collaboratively with HISD, as well as the District’s health care plan administrators to ensure they understand and can educate members on other health benefits available to them through the District Chosen vendor(s) will need to provide either online or telephonic phone scheduling of appointments and offer walk-in visits or same day appointments and will need to provide multilingual support to accommodate the District’s cultural diversity.
