Specifications include, but are not limited to: TASK 1 –Complete 2021 Annual Backflow Preventer Testing and CrossConnection Inspection•Utilizing a list of customers provided by the Westerly Water Department; contact, schedule, and perform backflow preventer tests at commercial and industrial users that were not yet tested / re-tested (if required) in 2021in accordance with the Rhode Island Department of Health and the Connecticut Department of Health regulations as applicable.•There are approximately175 testable deviceson the existing inventorythat still require annual testing/re-testingfor the 2021 calendar year. All tests shall be performed and reported to the Town by December 31, 2021.•Utilizing a list of customers provided by the Westerly Water Department; contact, schedule, and perform annual calendar year or five-year cross connection inspectionsat commercial and industrial users in Connecticut in accordance with the Connecticut Department of Health regulation.•There are approximately 37users in Connecticut that require annual calendar year cross connection inspectionsand 9 users that require5-yearcross connectioninspections.•Provide backflow preventer testing and crossconnection inspection forms acceptable to the Westerly Water Department.•Inform the Water Department of any non-responsive customers or customers refusing access after multiple attempts have been made.•Mail device test reports and cross connection inspection forms to property owner within 14 days of test/inspection. •Inform the Water Department within 30 working days, in writing, of any failed backflow prevention devicesthat have not been repaired or replaced and retested.Repair or replacement of failed devices is not includedas a part of the services being provided to the Town. Repair or replacement of failed devices is the responsibility of the user to be performed by the plumber of their choice.•Upon notification by the Town or the user that a failed device is ready for retesting, perform a retest of the failed device within 14 days of such notification.•Inform the Water Department immediately of any dangerous crossconnections discovered during inspections or testing. Follow-up in writing within 5 business days.•Maintain backflow preventer testing equipment and provide certification of calibration to the Water Department.•The Bidder must possess at the time of bid submission and submit with the bid a current valid Connecticut Department of Public Health Backflow Preventer Tester and a Cross Connection Survey Inspector certification for each individual performing testing and/or inspection.This certification must be maintained current for the duration of the award for services. Submit verification of tester and inspector certification to the Water Department annually.•If the Bidderfails to perform any of the aforementioned specifications according to State or Local standards, the Town reserves the right to terminate the contract at it's convenience with 30 days notice.•Invoices for the required tests and inspections performed alongwith the corresponding completed reportforms for each individual test or inspection shall be submitted to the Water Department on a monthly basis.•For information regarding the contract parameters, contractor relations with the Town and other Town contractors regarding coordination of the annual backflow preventer testing & cross connection inspection services contract please direct all such inquiries to the Sewer and Water Program Manager at (401) 348-2595or at apackhem@westerlyri.gov.
