Specifications include, but are not limited to: To provide more holistic care to individuals with behavioral health conditions, including, but not limited to SUD, SPMI, SMI, or co-occurring mental health disorders and SUD, the Delaware Department of Social Services, Division of Substance Abuse and Mental Health (DSAMH) offers Targeted Care Management (TCM) and Peer Support Services (peer support) to individuals with behavioral health conditions who need assistance with advocacy, mentoring, connecting with providers and treatments, qualifying for other public or community-based programs, and accessing other supports that are not being delivered by their primary behavioral health providers. In cases where care or case management is available through the individual’s current insurance provider, including Medicaid and Medicare, this program will serve as a short-term bridge to the insurer’s care or case management services. TCM will engage the individual, gather pertinent information to help inform the insurer’s Individualized Care Plan, and make a warm handoff to the insurer care or case manager.
