Specifications include, but are not limited to: The Contractor shall assist Vermonters who are applying and/or re-applying for General Assistance benefits in applying for Supplemental Security Income (SSI) and/or Social Security Disability Insurance (SSDI) benefits. The Contractor shall meet with individuals referred for application assistance; assist referrals with the collection of relevant information and documentation, which may include Personal Health Information (PHI) fill out forms, applications, and releases as needed; and submit SSA benefit applications and supporting documentation and materials using one of the authorized SSA submission processes. The Contractor shall follow each SSI and SSDI application to its conclusion, file appeals when appropriate, and communicate with the Social Security Administration (SSA) and the State’s Office of Disability Determination Services (DDS) on behalf of each applicant, as specified in Section D. Specifications.
