Specifications include, but are not limited to: The selected Vendor must identify HIV positive individuals who are newly diagnosed, recently relocated to NH, and currently not in care for HIV in order to provide the identified individuals access to the four (4) components of care for HIV early intervention services, as identified in the Health Resources Services Administration (HRSA) HIV/AIDS Bureau (HAB) - Policy Clarification Notice (PCN - 16-02). The selected Vendor will implement or expand an HIV Early Intervention Service (EIS) Program, ensuring all services are provided in accordance with the current Standards of Care for New Hampshire HIV/AIDS Services (NH SoC) and current the Centers for Disease Control and Prevention (CDC) HIV Testing Guidelines. The selected vendor will conduct Community Outreach to engage providers in order to identify individuals in need of services.
