Specifications include, but are not limited to: Providing assistance to approximately 100 individuals annually referred by State to apply for Social Security Disability benefits by performing the following functions: 1. Prepare and submit all Social Security application materials; 2. Schedule SSA interviews; 3. Communicate application status to the SSA and other State entities on behalf of applicants; 4. Follow the progress of each application and communicate with all parties; 5. Assist the applicant in appeals process or ensure legal representation; and 6. Provide successful applicants with information about State services, including benefits counseling. Services performed will be as ‘Fee for Service’ and Contractor will invoice State for services as follows: Service #1: Complete & submit Social Security Disability Application. Service #2: Complete & submit Social Security Disability Appeals Documentation. Service #3: Perform Social Security Disability Interview with Consumer.
