Specifications include, but are not limited to: Services that the successful contractor(s) will provide include the following: Personnel will receive the appropriate supervision from within their organization, as well as direction from DPH. Each staff member should have access to certified interpretation/translation services when necessary. It is essential to the success of the program that patients experience a smooth transition from one facet of the program into another, and that the personnel work collaboratively to share information. Additionally, personnel must utilize the web based SFL/HCC Database to submit client applications and supporting documentation. During year 1, the program will be exploring the movement to a pay for performance-based model that will utilize a CPT code to reimburse for each program application submitted. If successful, this will be available to all Screening for Life/Health Care Connection providers who submit program applications by submitting a claim for each client beginning in FY22. Bidders must demonstrate capacity to perform the full range of services listed in the Request for Proposal upon signature of the contract and throughout the term of the contract.
