Specifications include, but are not limited to: The purpose of this RFP is to solicit proposals to assist the staff members of the Heart Disease and Stroke Prevention Program (HDSPP) and the Diabetes Prevention and Control Program (DPCP) within the Office of Chronic Disease Prevention and Health Promotion (OCDPHP) in the Department of Health (DOH) to decrease the incidence of heart disease, stroke and diabetes through evidence-based interventions. The successful applicant will present innovative programming ideas that utilize proven strategies to change the health behaviors of all South Dakotans. They will also be able to demonstrate an ability to work with staff on the current priorities from the Centers for Disease Control and Prevention (CDC) which are Diabetes SelfManagement and Education and Support, prediabetes identification and referral, National Diabetes Prevention Programs, lifestyle change programs, community health workers, evidencebased quality measurement, team-based care, pharmacy services, self-measured blood pressure monitoring, and referrals to community resources and programs. A keen understanding of public health is a necessity. The applicant selected will be expected to help develop ideas for programs in both heart disease and diabetes and will need to have the ability to work with health care professionals, businesses, and individuals.
