Specifications include, but are not limited to: The selected Vendor shallconductstatewide intake, eligibility determinations, and other services in accordance with NewHampshire Administrative Rule He-M 500 Developmental Services, Part 520,Children’s Special Medical Services, which includes, but is not limited to, rules pertaining to:3.2.1.1.Application procedures.3.2.1.2.Program eligibility requirements.3.2.1.3.Services provided.3.2.1.4.Financial eligibility requirements.3.2.1.5.Payment for health-related services.3.2.1.6.Limitation of services.3.2.1.7.Appeals.3.2.1.8.Waivers.The selected Vendor must ensure applications are completed in accordance with He-M 520.02 and eligibility determinationsarecompleted prior toservice provision.The selected Vendor will provide home or community-basedassessments, intervention recommendations, and ongoing monitoring of the growth and health status of children and youth accepted for services by the Comprehensive Nutrition, Feeding andSwallowing Network (NFSN) andthe Bureau for Family Centered Services(BFCS).
