Specifications include, but are not limited to: Vendor shall implement the MFPTI project, which will include meeting with elder groups to share information and education about HCBS services; with technical assistance from a Mandan, Hidatsa, and Arikara (MHA) Nation elder, developing respectful elder training; meeting with elders and assessing for eligibility of HCBS services; completing care consultations and home and safety evaluations; providing introduction for HCBS Care Managers and assisting elders with program enrollment applications; advertising, recruiting, interviewing, hiring, and orientating new agency staff; providing services to eligible elders; completing quality assurance visits; monitoring and summarizing pilot project data; submitting reports to MHA Nation Councilman Spotted Bear and HHS Committee at 3- 6- and 12-months; submitting monthly reports to STATE and MFPTI project manager; submitting monthly SFN 1763 to STATE MFP project director; and attending national training.
