Specifications include, but are not limited to: The Contractor must: Provide IL and support services for all referred youth/young adults of eligible age as defined in IDAPA 16.06.01.011.12 and case management services for foster youth seventeen to eighteen (17-18) years of age for transition from foster care. Administer the IL Ansell-Casey Assessment instrument (http://caseylifeskills.force.com/) with Department referred youth, foster parents and others as requested by the Department. Facilitate the completion of the IL Ansell-Casey Assessment instrument within thirty (30) calendar days of the Department’s referral (if needed), involving the youth, foster parent(s) or other individuals as requested by the Department. Coordinate services and activities to facilitate completion of the Department referred youth’s individual service plan. The Contractor must also: Participate in a transitional meeting with the Department at the referral point of each case. Facilitate access for IL funds via the Department’s regional IL coordinator, as necessary and appropriate.
